First step: Download the Form 

For Wavier Case Managers or MSHO/MSC+ Care Coordinators: 

• If the Client has a waiver, attach CSSP or CCP alongside the Referral Form

For Targeted Case Managers:

• If the client is not on a waiver. 
• Obtain a Professional Statement of Need (DHS 7122) signed by a qualified professional.
• Complete a Housing-Focused Person-Centered Plan (DHS 7307).
